In Network

I am an in-network provider with Anthem/Blue Cross Blue Shield insurance. If I am in-network with your insurance plan then you will have a copay, coinsurance, or deductible which is determined by your individual insurance plan.

Out of Network

I am an out of network provider with all other insurance plans. However, I can provide documentation to submit to your insurance company. Many plans have out of network benefits and you may be able to be reimbursed for a portion of your visits. My rate is $150 for Individual Psychotherapy sessions.

Questions to Ask Your Insurance Company

• Ask about your copay, coinsurance, and deductible

• Most plans do not require pre-authorization for outpatient mental health benefits but it may be good to confirm this

• Confirm whether your mental health benefits are under Anthem/Blue Cross Blue Shield (some Blue Cross/Blue Shield plans "carve out" their mental health benefits to another plan)

• If applicable, you can ask if you have out of network benefits and how much this covers
